The Basic Forms

To run a successful tutoring service, it's essential to have good forms. The Basic Forms presents models of the Student Report Card, the Student Accounting Sheet and the Student Information Sheet. Please print copies of these forms and use them in your own tutoring lessons.

Use only one Student Report Card per month for each student to record all lessons with that student for the month. So, you'll nee six report cards to report six active students. Fill out all the information requested. Don’t skip spaces between lessons. Fax us or email us copies of all your Student Report Cards  NO LATER THAN THE LAST DAY OF THE MONTH.

Here are some hints to fill out your Student Report Cards  correctly.

  1. In the Date column, write the date of each lesson you gave the student during the month covered by this Student Report Card.
  2. In the Start Time column, write the starting time of the lesson.
  3. In the Hours column, write the number of hours the lesson lasted.
  4. In the Hourly Rate column, write the rate per hour you charged for the lesson.
  5. If you were paid by check, write the check number in the Check Number column. If you were paid by money order, write M.O. in the Check Number column.
  6. In either case write the amount of the check or money order in the Check Amount column.
  7. If you were paid by cash, write the amount in the Cash Amount column.
  8. Have the parent (or, if necessary, the student) sign in the Parent's or Student's Signature column.
  9. You sign in the Tutor's Signature column.

On the last day of the month, add up the numbers in the Hours  column, the Check Number  column, the Cash Number  column and the Total For This Lesson: column and write the sums in the bottom line, after the work Totals:.

On the last day of the month, don’t forget to check the Yes box at the end of line two if you believe the student will continue having lessons with you next month or the No box if you believer that lessons will not continue next month. YOU MUST CHECK ONE OF THESE TWO BOXES. If you’re not sure, make your best guess! Remember, however, that if you check the Yes  box, you must include a report card for this student in next month’s end-month mailing, even if the student does not take any lessons with you next month. If you check the No  box, then this is the last report card we expect to receive for this student.
